Eligibility Requirements

Patient is an Alameda County resident AND
Patient is an U.S. citizen or have been a legal permanent resident for at least five years AND
Patient's age 19 to 64 AND
Patient's income is 0-200% (up to $1,862 per month for a single person) of the Federal Poverty Level AND
Patient is not eligible for, or enrolled in Medi-Cal AND
Patient is not pregnant
