- You are a resident of the State of Arizona
- You have private insurance or Medicare
- You have an HIV diagnosis
- You cannot be eligible for AHCCCS (Arizona's version of Medicaid)
- ADAP will only process prescriptions written by a licensed Arizona physician/prescriber
- Your annual income is set at 300% of federal poverty level, refer to the 2012 HHS Poverty Guidelines Click here)
